2011年2月に、右股関節の"最小侵襲人工股関節手術 : MIS :
Minimally invasive Surgery, (MITHA : Minimally Invasive Total Hip Arthroplasty)".
手術前、軟骨がすり減り、骨盤と大腿骨が直接触れ、右大腿骨は細くなり、痛み止めを飲んでも痛みはおさまらず、非常にゆっくりと歩くことしかできませんでした。
手術は無事終了しましたが、右足は2cm長くなってしまっていました。

Before the operation, my right cartilage had worn down, my pelvis and my thighbone were
stuck and even when I took painkiller I felt pain so I walked very slowly.
The operation almost succeeded, although the only problem was that the right leg was 2cm longer than the left leg.
A doctor said when I have the operation for my left leg, they will make them the same length.
The pain in my right leg has gone, although my left leg started hurting gradually.
2) 2012年9月 / September 2012
今回の左股関節の手術は、同じ病院で、ほぼ同じ手術で、私の希望は左股関節の痛みがとれるとともに、足の長さがそろうことでした。
左股関節は、右股関節の手術のときよりも、かなり良い状態でした。
手術直前まで、杖をともないながらも、結構な距離を通常の速度で歩くことができていましたし、骨も右足のように細くなっていませんでした。
きっと筋肉もさほど落ちていなかったのではないかと思います。
The Left Hip

I could walk quite a long way at a normal speed with a stick, and the bone has not become as thin as the right leg.
Also I think I have not lost as much muscle from my left leg.

B. 27日 / 27th
はたして.......、手術は無事終了し、右足が0.5cm長いほどになりました。
この程度は、一般的には許容範囲らしく、無理に足の長さをあわそうとすると、足がしびれる等の原因になるそうです。
私はもともと低血圧です。
前回の手術のとき、一層低血圧になってしまいまいました。

今回は、麻酔を調節することによってその問題はほぼ解決されました。
今回は、詳細ながらも、様々な点が変更されており、それらは改善された結果だと信じたいです。
27th
Sure enough....., the operation almost succeeded though my right leg is now 0.5cm longer than the left leg.
I heard that generally the difference is within a permissible range and if they forcibly make the legs the same length, the leg might become numb or have other problems.
Generally my blood pressure is lower than average.
This time, they changed the anesthetic method and it was successful.
When I was moved from the operating bed to the bed in the room after the previous operation, one of the tubes came off and my blood splashed in the room, although this time, the lifting was done safely.
Various details were changed .

術後, 肺塞栓症を防ぐ
1) 段階圧力ストッキング (写真内：白いソックスに見える)
シャワー時以外24時間着用、2ヶ月間続けます。
このストッキングは手術をして部屋に戻って来た時には、
既に着用しています。
2) 血流を促すフットポンプを24時間足につけます。
2日めから退院まで4時間。 After The Operation, Venous Trombosis Prevention
1) We should wear pressure stockings (in photograph, they look like
white socks), for 24hours a day (except for having shower) for two months.
When patients come back to the room from the operation,
they (we) already wear the stockings.
2) We wear 'foot pumps' for 24 hours.
From the second day after the operation, it becomes 4 hours a day until leaving hospital. 脱臼を防ぐ
手術した足が内股になる(内側に入る)と脱臼の危険があるので、
両足の間に障害物(写真参照)を置きます。
2日めからはもう少し小さなクッションに交換。
Preventing Dislocation
Dislocation is dangerous, and could easily happen if the operation leg points inwards, so an obstacle (see above photograph) is put
between our legs.
From the second day, it becomes a smaller cushion.

C. 最初の一歩 / First Step
術後初めて床を踏む左足は、右足の時よりも、非常にしっかりしていて、自分でもびっくりしました。
そしてとても明るい気持ちになりました。
First Step
After the operation, from my first left step on the floor I felt my left leg was so much stronger than my right leg had been after the first operation, so I surprised myself.
After that I have such a bright feeling.

手術翌日から使用する歩行器
飲み物の自動販売機がある2階に行こうとしたところを、
まだはやすぎると、看護士さんに止められました。
ナースステーションでは、このような無謀なこと？をする
(元気すぎる) 患者として話題になったそうです。
This walker is used from the first day after the operation
I was trying to go downstairs to a drinks vending machine, although a nurse stopped me, because it was too early or ambitious. I heard that in the Nurses Station, it was a popular tale that one of the patients would go downstairs, even though it was the first day after the operation.
After the second day, we start using a stick rather than the walker.

以前感じた平川先生は監督者という立場よりも、今回の手術は、
担当医として動いていらっしゃるのを感じましたし、術前後を通して直接彼と話す機会も多かったです。

傷口をとめるテープもきれいに張られ、それがはがれるのも前回よりも早く、手術の傷口も細くまっすぐときれいでした。
平川先生がメスを入れたのではないかと推測しますが、もしかしたら数多くの手術経験を経た他のドクターの誰かが腕をあげて、おこなったのかもしれません....???
Operation
When I had the previous operation, I felt Doctor Hirakawa, who is called 'Super Doctor' in Japan, oversaw my operation and others, however, this time, I felt he worked directly for me and other patients?
Before, through, and after the operation I talked with him much more often than for the previous one.
The tapes across my wound were neat, they fell away earlier than for the previous one and the wound is thinner, straighter and not puckered.
I guess Dr. Hirakawa performed the operation on my leg , although maybe it was done by one of the other doctors who now has more experience and their skill and technique has improved...???

全体の流れとして、ヒューマンエラーを防ぐシステムが増えていました。
例えば、バーコードの使用(下の写真参照)。
さらに言えば、前回にはなかった、術後の注意事項などについての説明会が、手術前日に行われていました。

点滴
抗生物質の点滴は手術前述から術後３日目の夜まで、
朝晩一度づつ行われます。

カーテル

バーコード
以前もリストバンドはしましたが、今回はバーコード付き。 点滴時、熱、血圧などを測るときにバーコードをチェックされ、 コンピューターにデータが送られます。
リストバンドは医療ミスをかなり防ぐと思います。
Bar-code I had a wrist band before, although this time, a wrist band with a bar-code. When I had a drip, a nurse checked my body temperature and blood pressure, and used a bar-code reader which fed data into their computer.
I think the wrist band protects against medical error well.

食事は、改善されていることを感じました。

4) 窓口、会計部門

ここは、窓口、会計部門は、相変わらず、他の部門に比べると、気配りが劣ります。
マニュアル内にあることは、できるけれど、そこから外れると患者に対してクールな対応しかできず、マニュアルにあることを通そうとすると私は感じていました。(なかには患者の意向を優先させてくださる方もいますが)
また、他のスタッフの間違いをかわって謝る事もできません。
具体的な仕事 (伝えられていた期日までに書類が整わない、退院時間に会計が間に合わない等) にしても、あまりあてにできません。
マニュアル内にあることは、できるけれど、そこから外れると患者に対してクールな対応しかできず、マニュアルにあることを通そうとすると私は感じていました。(なかには患者の意向を優先させてくださる方もいますが)
また、他のスタッフの間違いをかわって謝る事もできません。
具体的な仕事 (伝えられていた期日までに書類が整わない、退院時間に会計が間に合わない等) にしても、あまりあてにできません。
退院して一週間たつのに、まだ各証明書が届きません。
地方都市の病院で、人材不足で仕方がないことなのかもしれませんし、何か他に問題があるのかもしれませんね???
ですが、私の経験には限界があるので、私の印象は、正しいことではないかもしれません。

入院中のスケジュール
各自にこれが渡されます。以前の倍のサイズになりました。 Schedule of Hospital Stay All patients have this. The size has doubled. |
Here, as usual, the reception and administration area are much worse than other sections, in their lack of consideration for patients.
I felt that if they follow the sequence of their work in the manual they could do their job, however, if the work is not in the manual, or is out of sequence they become cold reception for patient(s), so that they can stick to their sequence. (One receptionist prioritizes the patient's needs.)
And also they could not apologize about other staff's mistake instead of the staff.
Their word cannot be relied upon.
For example, they cannot prepare documents which they said would be ready when I would leave hospital.
One week after leaving hospital, I haven't got my medical certificates yet.
I think they cannot help it in a local hospital with a shortage of good staff, and there might be other reasons, mightn't there???
I feel like this, because in my experience, the staff in Ito Hospital in Aoyama in Tokyo and Juntendo University Hospital in Ochanomizu in Tokyo are so much better.
My experience is limited, so my impressions might be not correct.

5) 部屋 / Room
私の部屋は、前回は2人部屋、今回は個室:108号室でした。

この病院は、個室11床、緊急用個室１床、2人部屋8床なので、個室になるのは特別なことではありません。
いろいろな面で好き勝手できるので、私は個室の方が好ましいですが、なかには寂しいから2人部屋の方が良いという方もいらっしゃるようです。
Room
Before I was in a twin room but this time, a single room, 108.
This hospital has 12 single bedrooms, including one for emergencies, and 8 twin rooms.
So it is not special for somebody to have a single room.
I can do things as I like, so I prefer having a single room to a twin room, although some people prefer twin room to single room, because they feel isolated in a single room.

ここの電話は外からかかってきた時にだけしか使用できません。
携帯電話の使用もPCの使用もできますし、 もともと私はTVは見ないので、 私には両方とも必要ありませんでした。 それを知ったH婦長さんがテレビと電話を とりはずしてくれたのです。
私はこのH婦長さんにはちょっと感動しました。
上記の事だけでなく、患者(私)への注意を促す時も、
その表情にも言葉選びにも心を傷つけない配慮がありました。
さすが婦長〜〜！
(私はあえて婦長と言わせて頂きますが
今時は'婦長'という役職でなく、'総看護士長'と呼びます)
他の看護士さんも親切ですよ！お忘れなく！Here, the telephone can only be used for incoming calls. I could use my mobile phone and my PC there and as I do not watch TV, I did not need their TV, or the phone. H Chief Nurse knew it, then she took them away.
By the way, I was impressed by H Chief Nurse,
not only the above but also other things that
she told me and the manner that she used,
her look and her choice of words would not hurt my feelings.
You are my Star.
Don't forget that other nurses are kind, too. |

2011年のリハビリ中に使用した靴
靴底の高さの違いが足の長さをカバーします。
2,3週間貸し出してくれます。
その後は自分の靴のかかとに、
自分にあった高さの'中敷き'(これが結構高い！)を
入れて調整しました。
今回は必要ありませ〜〜ん。
I used them when I had rehabilitation in 2011 The different heights of the soles helps walk with different length legs.
They lend them out for two or three weeks.
After that I can only use a wedge-shaped inner heel
for my shorter leg.
This time, I do not need them.

細菌の感染や脱臼をしないように生活を心がける毎日です、もしくは永遠にかな？
そしてまだアルコール類は飲めません....(涙)

I understand that the reason that they tried to make the same length then they stretched my left leg (he~~lp!) in the operation, so it cannot help it.
I heard from a physiotherapist that if my muscle become softer, my leg would feel better.
Now I should guard against bacteria and always be careful not to dislocate, forever?
And also I cannot have alcohol yet.....(tears)
Oh! Yes! R came back from Britain.
